Summary:

 Our Disease Management (DM) Programs provide a comprehensive, coordinated, and proactive approach to    managing care and support for members who are enrolled in the Medicaid STAR program and the Seton Employee  Plan and who have chronic and catastrophic illness, including asthma, diabetes, high risk pregnancy, and special healthcare needs.

Our nurses provide:

  • extensive patient education to help our members better understand their disease process

  • collaboration with community agencies to provide the extra care our members need

  • development of a coordinated plan of treatment with the our members and their physicians

Seton’s Disease Management RN works in an office-based setting and typically contacts the patient by phone. Participation in the program is voluntary. If the patient agrees, the Disease Management RN performs an initial assessment similar to the first visit to a physician’s office. Using standardized evidence-based guidelines, the Disease Management RN discusses the recommendations for medical care and lifestyle behaviors that reduce complications and produce the best outcomes, working closely with provider offices.

Education and Experience:

Two (2) years of nursing experience in an acute care or surgical setting is required.  Experience in utilization review, knowledge of managed care rules and regulations, government policy rules and regulations, and computer skills are preferred.

Bachelor’s or Master degree in Nursing preferred.
NOTE: By January 1, 2020, all associates in this job title must have completed a Bachelor’s of Science in Nursing (BSN) or higher nursing degree.

Licensure/Certification/Registration:

Registered Nurse (RN) licensed to practice in Texas is required or must be obtained prior to hire or job transfer date.
